The article compares the effect of inhaled and intravenous prostaglandins on postoperative period in
patients whose were operated for acute pulmonary embolism. 49 patients were included in the study and
were divided into three groups: the first group – 15 patients who received inhalations of iloprost (Ventavis,
Bayer Pharma AG, Germany); the second – 16 patients who received intravenously alprostadil (Vazaprostan,
UCB Pharma GmbH, Germany); the third – 18 patients who received standard postoperative intensive care.
The use of prostaglandins after pulmonary thrombectomy provided a more effective level of correction for
residual pulmonary hypertension compared with the standard protocol of intensive care. The effectiveness
of the inhalation form (Iloprost) is higher than that of the intravenous (Alprostadil), and is accompanied by
faster normalization of the pulmonary artery pressure level, pulmonary vascular resistance, a higher oxygenation
index after the operation, and an earlier cessation of mechanical ventilation.
