Highly sensitive troponin in diagnosing miocardial infarction: a real diagnostic value or overestimated capabilities?

Early diagnosis of myocardial infarction (MI) is the milestone of prevention of early complications and mortality.

However, his diagnosis in the case of a combination of typical anginal syndrome without ST-segment

elevation in the ECG is a difficult task in the prehospital and early hospital stages of the management of

patients with acute coronary syndrome (ACS). The introduction of highly sensitive cardiac troponin

(hs-сTn) was able to increase the frequency of detection of MI up to 47% in patients with ACS without

ST-segment elevation. However, despite the obvious advantage of hs-Tn allowing cardiomyocyte necrosis

noted in the beginning of the disease, the cost of the high sensitivity of the method was the decrease in its

specificity, and the related difficulty of interpretation. The aim of this research is the analysis of the available

literature on the use of hs-сTn– the modern biomarkers of necrosis of cardiomyocytes.
