ИКД-терапия на современном этапе: возможности в снижении неадекватных шоков

Аннотация

В статье рассмотрена ИКД-терапия как способ сохранения жизни пациента с высоким риском внезапной сердечной смерти и как фактор, влияющий на общую смертность. Определена негативная проблема воздействия ИКД- терапии: нанесение неадекватных шоков. Проведен анализ причин нанесения необоснованных воздействий дефибриллятора. Рассмотрены пути возможного уменьшения количества нанесенных дефибриллятором шоков. Определены причины необходимости нанесения именно шокового воздействия. Предложены параметры программирования детекции тахикардии и терапии тахикардии для замены шоковых воздействий на антитахикардитическую стимуляцию. Проведен анализ использования современных алгоритмов ИКД для уменьшения нанесения шоков. Актуализирована необходимость продолжения рассмотрения вопросов показаний, своевременности имплантации ИКД- устройств, их функционирования.
